What is a DOT Physical?
A DOT physical is a medical examination required by the Federal Motor Carrier Safety Administration (FMCSA). It's performed by a certified medical examiner listed on the FMCSA National Registry. The exam checks that you can safely operate a commercial motor vehicle.
What the Exam Covers
Vision
- Distance: At least 20/40 in each eye (with or without correction)
- Peripheral: At least 70 degrees in each eye
- Color recognition: Must distinguish traffic signal colors
Hearing
- Must hear a forced whisper at 5 feet in at least one ear (with or without hearing aid)
- Or pass audiometric test at specific decibel levels
Blood Pressure
- Stage 1 (140/90 or below): 2-year certification
- Stage 2 (140-159/90-99): 1-year certification
- Stage 3 (160-179/100-109): One-time 1-year certification, must be recertified
- Stage 4 (180+/110+): Disqualified until treated and below Stage 3
Urinalysis
- Tests for sugar and protein (diabetes and kidney indicators)
- This is NOT a drug test (that's separate)
Physical Examination
- Heart and lung function
- Neurological check (reflexes, balance)
- Musculoskeletal assessment (range of motion, grip strength)
- Abdominal examination
- General health assessment
Disqualifying Conditions
Certain conditions will prevent you from passing:
- Uncontrolled diabetes requiring insulin (waiver available)
- Epilepsy/seizure disorder (waiver available after 8+ seizure-free years)
- Severe hearing or vision loss beyond correctable limits
- Uncontrolled high blood pressure (Stage 4)
- Current substance abuse
- Certain cardiovascular conditions
- Sleep apnea (if untreated — CPAP use is accepted)
How to Prepare
1 Week Before
- Check blood pressure: If it's borderline, reduce sodium, increase water, exercise lightly
- Gather medications: Bring a list of all prescriptions with dosages
- Get sleep: Consistent sleep in the days before helps BP and alertness
- Reduce caffeine: Caffeine spikes blood pressure temporarily
Day Of
- Eat light: Avoid heavy meals before the exam
- Stay hydrated: You'll need to provide a urine sample
- Bring documents: Photo ID, glasses/contacts, hearing aids, CPAP documentation
- Bring medical records: If you have a treated condition (sleep apnea, diabetes), bring proof of treatment
What to Wear
- Comfortable clothing that's easy to move in
- Short sleeves help with blood pressure checks
The Exam Process
- Paperwork: Fill out the medical history form (be honest — lying can result in license revocation)
- Vision test: Read the eye chart
- Hearing test: Whisper or audiometric test
- Blood pressure: Measured at rest
- Urinalysis: Standard urine sample
- Physical exam: Full body check by the examiner
- Results: Same-day pass or fail. If you pass, you receive your medical certificate
Cost
DOT physicals typically cost $75-$150. They are NOT covered by most insurance plans. Some truck stops and urgent care clinics offer competitive pricing.
How Long is it Valid?
- Standard: 2 years
- With conditions (controlled BP, diabetes): 1 year
- Temporary: 3-6 months if examiner wants to monitor a condition
After You Pass
